Our Vaccination Requirement

Northwestern requires COVID-19 vaccination for all students who have a presence on campus.

Students must submit proof of their primary vaccination series or obtain approval for an exception in order register for classes at Northwestern.

Vaccine Boosters

The University encourages booster shots.

Northwestern does not require a booster dose to be compliant with the COVID-19 vaccination requirement. We strongly recommend boosters for those who medically are able to obtain them. They are important individual tools for minimizing severe illness.

Report Your Vaccination

Complete the COVID-19 Vaccination Disclosure form to report your vaccination or request an exception.

  • Students who seek an exception to the University’s vaccination requirement due to a personal health condition, a sincerely held religious belief or lack of access to an FDA- or WHO-approved vaccine must submit a formal request at least two weeks prior to course registration.
  • Students also may include their booster information and documentation.
  • If an employee wants to enroll in a credit-bearing course on campus, they are required to submit a COVID-19 Vaccination Form, given their role as a student.

Students, faculty and staff should take advantage of the free walk-up appointments offered by pharmacies such as WalgreensCVS and Jewel-Osco. Students also can make primary series appointments at Student Health by scheduling via MyChart. Patients of Northwestern Medicine can make primary series appointments at select NM Primary Care locations. If you still need assistance making a COVID-19 primary vaccination series appointment, contact the vaccine support team by emailing vaccine@northwestern.edu.
